NABC Convention PDS Certification Entry Form

To earn NABC PDS certification, members must attend at least five clinics during the NABC Convention. All sessions labeled “PDS Clinic” on the NABC Convention schedule count towards PDS Certification. At the conclusion of the NABC Convention, members who met the requirements can fill out the following form to confirm PDS certification.
